Alaire Psychiatry, PLLC is an In-Network provider of Blue Cross Blue Shield PPO, and BCBS Blue Choice PPO insurance networks. Some Blue Cross Blue Shield plans are administered by a third party administrator, and we may not be in network with those specific plans. We recommend contacting your insurance carrier to verify your coverage and benefits before scheduling your first appointment.
Appointment types and Rates for Out-of-Network care/ Self-Pay
If you have a different health insurance plan, you will be considered out of network and will be asked to pay for your services at the time of your visit according to the fee schedule below. You can receive a superbill or invoice that you can submit to your insurance carrier for out-of-network reimbursement according to their rates and your insurance deductible.

Full diagnostic psychiatric evaluation.
Thorough review of your presenting problem, symptoms, complete psychiatric history, medical history and contributing factors.
Determine diagnosis and start to discuss a treatment plan or treatment plan changes.
Assess alignment and whether we will move forward with a doctor-patient relationship.

Full diagnostic psychiatric consultation above and creation of a specific treatment plan and recommendations.
This will be recommended for a ONE TIME CONSULT or SECOND OPINION CONSULT where you need to take recommendations back to your pre-existing physician or provider.
This may also be recommended in specific cases of high complexity. You would have communication in advance of scheduling if this extended consult is recommended.

Once an initial evaluation is complete, the rest of your visits will be follow-up visits. We will decide on treatment plans, have discussions about risks versus benefits, monitor progress, order and review blood work when applicable, adjust or change medications as needed.

Offered as requested to pre-existing patients who prefer to have more time for integrated psychotherapy work in addition to their targeted medication management.
Fees for Disability Forms/Other Forms/Letters
If paperwork is beyond what can be completed during the face to face appointment or is requested to be completed outside of a visit, fees will be as follows (unless otherwise provided to you with an alternative estimate). This applies for patients who are In-Network and Out-of-Network/Self-Pay.
Short Term Disability Forms. . . . . $200 (includes the correspondence with the disability carrier)
Attending physician statement/Letter with Treatment Summary for Life Insurance Company . . . . . $100
Accommodations Forms. . . . . $50
Letter for Emotional Support Animal for housing. . . . . $150 (includes correspondence and changes necessary). This is offered to patients who are established only, consults for this purpose only are not taken. Please note letters for ESA for travel are not provided.
Prior Authorization Appeal. . . . . $100 (in the case a standard prior authorization is not accepted, a lengthy appeal process may be required in order to try and get medication approval).
Miscellaneous Forms or Letters not otherwise described above - estimate will be provided on request based on time required for letter completion.
Cancellation & No Show Fees
Once an appointment is scheduled, any cancellations or reschedules must be made at least 24 business hours in advance. Business days are considered weekdays, and exclude holidays. Monday appointments, you must cancel by Friday at 4 pm.
Cancellations made less than 24 business hours prior to the appointment time or No Shows will be charged the following cancellation fees:

No shows are appointments that are missed without prior notification or cancellation.
It is considered a missed appointment when you are 15 or more minutes late to a scheduled appointment.
1 attempt will be made to reach you during your visit time as a courtesy reminder. You must join the appointment before it has been 15 minutes from your appointment start time.

3 or more no shows within 1 calendar year will result in termination of the patient-physician relationship and a termination letter will be sent.

One time fee exemption for established patients of the practice. All follow-up patients are given 1 waived cancellation/no-show fee.

No, no show/missed appointment fees cannot be billed to insurance.
Good Faith Estimate
Under the No Surprises Act, you have the right to receive a “Good Faith Estimate” explaining the expected cost of medical and mental health services. This estimate provides a clear, upfront breakdown of the fees for our services, helping you make informed decisions about your care.
If you’re choosing to self-pay (not using insurance), we will provide you with a Good Faith Estimate before your appointment in the intake paperwork. This estimate outlines the anticipated costs of treatment, so there are no surprises. Please note that actual costs may vary based on your specific needs and treatment plan and would be discussed specifically in that case.
