When you send your contact info in the below form, you will be sent an email with a secure log in to our brief inquiry form.
Following completion of the brief inquiry form, you will either receive a call once that is reviewed or sent a link to our full intake forms and will be contacted for appointment booking.
If leaving a voicemail, please include:
Your full name, reason for consultation, email address and your call back phone number where we can leave a voicemail
You will be called for additional information and when applicable, a HIPAA-secure online full intake form will be emailed to you and you will be contacted for appointment booking.
When applicable, you will be recommended to book one of the appointment types: an Initial Psychiatric Consultation (60 min) or Extended Initial Psychiatric Consultation (75 min) depending on your specific inquiry (see more info under our insurance and fees section, appointment types).
